There are 3 initial appointments in our office. You will learn a great deal about orthodontics, as it applys to your situation, during these 3 appointments. We invite you to ask as many questions as you would like at each step of the way.

Initial Consultation
Our warm and cheerful staff will welcome you as soon as you arrive at the office. You probably have a lot of questions about your orthodontic treatment. Dr. Gorin will perform a gentle examination that usually reveals a great deal about the patient’s orthodontic situation. If treatment is indicated, Dr. Gorin will need a little more information so that a comprehensive plan can be developed. All of the information that Dr. Gorin requires is obtained at the diagnostic records appointment. The records can be taken on the day of your initial consultation if it is convenient for you.
Your schedule is probably hectic. Filling out forms at a doctor’s office is just a waste of your time. That is why we have provided on-line forms for you. Click on either the adult or child links to download the appropriate initial consultation and HIPPA forms. Print the forms and fill them out when convenient. Please bring the completed forms to your consultation appointment.

Second opinions Dr. Gorin is often called upon to give a second opinion. Please bring duplications of your orthodontic records to the initial consultation appointment if you are seeking a second opinion.
Click here to download a Request for Duplication of Orthodontic Records document. This document is used to formally request duplications of your records from the previous orthodontist. Upon receipt of the document, the orthodontist will supply you with duplications of your records. Our office can also make a request for your records if you prefer. A complete set of orthodontic records includes: panoramic x-ray, cephalometric x-ray, models and photos.
Our office uses the information obtained from the diagnostic records to develop a comprehensive treatment plan. X-rays and molds of the mouth will be taken at this visit. In the old days the molds were poured into stone so that a representation of the mouth could be obtained. Our office utilizes state-of-the art technology which includes sending the molds to a scanning lab. The scanning process turns the information obtained from the molds into a digitized representation of the oral condition. The data is then uploaded to our computers so that Dr. Gorin can perform a detailed analysis. Specific landmarks on the x-rays are also digitized so that growth patterns, space availability and responses to proposed treatment can be evaluated. The records appointment takes about 30 minutes.
Case Presentation
The case presentation appointment is scheduled after Dr. Gorin has developed a comprehensive treatment plan, which is usually a few days after the diagnostic records appointment. This appointment is scheduled at the end of the day when the office is quiet and when Dr. Gorin can devote complete attention to you and the presentation of the plan. Treatment options and alternatives are discussed as well as appliance selection. Fees, payment options and insurance contributions can also be discussed at the case presentation appointment. Orthodontic treatment can proceed after the case presentation appointment.
